The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

Need Adobe Acrobat Reader?

If you do not have a copy of Acrobat Reader you can get one for free by clicking on the icon 'Get Acrobat® Reader'. This will take you to Adobe's website and enable you to download the Acrobat program to your computer.

Payment/Financing Options

It is our mission to provide you with affordable, high-quality services that fit your budget! We offer a variety of payment options to meet your needs. For your convenience, we accept VISA and MASTERCARD. We also offer Third party financing with Health One Financial and Med Loan Finance.

Photo Privacy

Photographs are an extremely important part of any plastic surgery practice. Dr. Austin takes digital photographs to track a patient's progress before and after surgical and nonsurgical treatments and procedures. To protect the privacy of his patients, Dr. Austin does not post any of these photographs online. Many of his patients have signed consent forms giving us permission to uses these photos for educations purposes. Therefore, before and after photographs of some of our patients are available in our office to be viewed during your consultation with Dr. Austin in the privacy of an exam room.

Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hour notice. This courtesy, on your part, will make it possible to give your appointment to another patient.
